How to apply
When applying for an application, you will need to provide a copy of your public liability insurance and provide a scaled plan of the premises which includes the following:
- ambit of the licensed premises
- defined treatment areas and rooms
- exits and escape routes
- WC's, showers, wash hand basins
- passageways
- general location of equipment

Unlike postal applications you are not required to send copies of your application to the police or fire authority.
You will need to ensure payment is made with your application. You can view the relevant fee below.
Other application types
Transfer of licence holder
The licence can be transferred from one holder to another. A transfer application must be made and the appropriate fee paid. Consent from the outgoing licensee must be submitted with the application. If consent cannot be obtained from the outgoing licensee then proof that all reasonable attempts have been made to get their consent. You can make this type of application below.
Variations
Once a licence has been granted, it can be varied in a number of ways including adding licensable treatments or changing the licensable area. You can make this type of application below.
Registered therapists
The Council used to require all persons providing licensable treatments to register with us, however this is no longer the case. Westminster no longer requires therapists to be registered by the Council and asks that licence holders ensure that their staff are qualified to provide the relevant treatments.
Application forms
You can apply online for the following:
- New special treatment premises licence
- Renew your special treatment premises licence
- Vary, transfer, change licensee details or surrender your special treatment premises licence
Download the statement of consent from the licensee to transfer template document
To request a copy of a postal application form, please email the Licensing Service at [email protected] stating the application form that is required.
Please note that the Council has established a cheque payment surcharge for all Type 3 and some Type 2 licensing regime fees. For further information on these licence types, please view the fees list above.
Eligibility criteria
If objections are made and an agreement cannot be reached to conditions that satisfy all parties, the case will be referred to a Licensing Sub-Committee to be determined. It may decide to refuse to grant, renew or transfer a licence.
