Library service user research participation form

Westminster City Council and Royal Borough of Kensington and Chelsea are working with digital agency dxw on a research project to understand how the public read, learn and connect with their communities.

The research session

Our phone conversation will be around 30 to 45 minutes long. During the call we will ask you questions about your experiences with reading, learning and connecting with others.

Taking part in research is voluntary. You can take a break or stop at any time.

Collecting research data

Westminster City Council and Royal Borough of Kensington and Chelsea may make sound recordings and take detailed notes of your research session. Westminster may also make a complete transcript from the recordings.

Westminster City Council, Royal Borough of Kensington and Chelsea and dxw staff will be able to review and use the research data.

We will store any research data we collect securely for up to 1 year.

Observing the research

Westminster City Council, Royal Borough of Kensington and Chelsea and dxw staff may listen to your research session, through a conference call link.

Sharing results

Westminster City Council, Royal Borough of Kensington and Chelsea and dxw will share the results of the research with stakeholders. Westminster City Council, Royal Borough of Kensington and Chelsea and dxw may make some of these results public. The results we share and make public will not contain any of your personal data or anything that could identify you.

Consent

By completing and submitting this form, you agree to take part in the research session and you consent to the personal data you (audio recordings and notes) give being used to help dxw, Westminster City Council and Royal Borough of Kensington and Chelsea understand the needs of people wanting to read, learn and connect. Personal information including your name, address and contact details will not be shared with anyone.

Using your personal information

The information you have provided will be used by the Council’s Web Services Team, Royal Borough of Kensington and Chelsea's Web Team and DXW to process your request to participate in the Library Service user research. 

The council will not use the information you provide for other unrelated purposes. The council will not share your data with any other third parties unless it is legally obliged or lawful to do so. This includes where it is deemed necessary for the prevention and detection of crime, the protection of the public purse or where there is a risk of serious harm or threat to life. The council will inform you of any changes to the processing of your data as soon as it is reasonably practicable to do so.
