- Medical letters or reports that you already have
- Prescriptions
- Hospital discharge summary
- Disability Benefit Award letter e.g. Disability Living Allowance, Personal Independence Payment or Employment and Support Allowance
- Patient summary (you can ask your GP for this if seeing them before completing the form)
