Which award category are you nominating in?

Criteria: this award is open to any adult who provides care and support to a friend or family member with mental health needs in their own home.

Criteria: any adult who provides care and support to a friend or family member with learning disabilities in their own home.

Criteria: any adult who provides care and support to a friend or family member with physical disabilities in their own home.

Criteria: anyone under the age of 18 who looks after someone in their family who has a disability, illness, mental health condition or addiction (this can be helping around the house, looking after siblings or taking on extra responsibility for someone else in their family).

Criteria: People who volunteer to provide care and support to others, through volunteering with a local organisation, a charity or the council. This may include being a befriender - often providing support and companionship to a range of people who may be isolated or have no family, usually by visiting them at home.

Criteria: anyone who provides care and support for someone over the age of 65 in their own home.

Criteria: a parent who provides care and support for their child or children.

Criteria: any person who has helped someone approaching the end of their life, by providing practical or emotional support.

Criteria: any adult who provides care and support to a friend or family member with learning difficulties in their own home.

Criteria: any person who provides care for more than one person such as a parent and child.
