Adult Social Care and Health

The London Borough of Hammersmith and Fulham, the Royal Borough of Kensington and Chelsea, and Westminster City Council are the first London local authorities to lead the way and join up Adult Social Care services across 3 boroughs.

Who we are

With all three boroughs ranked as having the best adult social care services in the country, we intend to maintain these high standards supporting people closest to their communities and homes and outside of acute and institutional care where it is appropriate. 

Our aim

Our overall aim across the three boroughs and in health is to put our customers first, ensuring they continue to receive good quality services, a satisfying care experience and achieve good personal and individual outcomes to exercise choice and control over their lives.

Our ambitions

We are focused on our work with health colleagues in primary care, secondary care and in community health to change the behaviour in how people look after their health in the future.

We want to see our customers being as independent as possible, using their own networks for support and receiving planned care and support wherever possible, with a focus on prevention and early intervention.

As part of this ambition, Adult Social Care and Health have plans to integrate further with health partners and deliver streamlined services to provide best outcomes for our customers. We aim to:

  • support carers in their caring role and recognise the importance of people’s relationships and families
  • personalise and promote health and wellbeing to people who need care and support

  • Personalisation – Offering flexibility, choice and control for customers, including carers
  • Integration – Social care,  housing and health working together to provide a better service for our customers
