What we do...
The job of a Community Learning Disability Nurse is to help people with a learning disability to get healthy and stay healthy.
When this is not possible, we help people cope with poor health and illness.
Some of the things that we do include:
- helping someone work out if their tablets are suiting them
- going with someone to the doctor or hospital
- teaching people how to stay healthy
- teaching and supporting carers to help them look after someone
- writing a Health Action Plan
We run a health promotion clinic once a week, and sometimes run health education groups.
Challenging Needs Nurses do these things too and also help people to understand about behaviour (link here to Challenging Needs Team).
Who we work with...
Community nurses work with adults with a learning disability who live in Westminster and need special help to stay healthy. Mostly we work with people 1-1, but sometimes we run groups, or do things which affect everyone with a learning disability.
We sometimes work with younger people who will be using adult services soon.
We work with families, carers, other health services, social services, and anyone else who is involved in supporting people with a learning disability to stay as healthy as possible.
