How to apply

The application pack consists of 4 forms which you can download here.

Please note that white disabled badge holders now have one month to renew their expired permits.

Fill out the necessary forms and send by post to:

Alternatively, if you require assistance completing the forms, you can visit one of our One Stop Services.

What else do I need to send? 

• 3 passport photos signed on the back by the applicant (if you don't need a blue badge as well, you only need to send 2 photos)
  
  
• A copy of the vehicle registration document for each listed vehicle (if the applicant is the owner and driver of the vehicle, the V5 must be registered to a Westminster address)
  
  
• Appropriate disability proofs as detailed in the application form.
