Supported Self Assessment Questionnaire. This questionnaire is a new assessment document which allows you to tell Adults Services about your living situation, how you currently manage on a daily basis and any needs you feel you have that you may require some support with. As part of the assessment process, Adults Services will then arrange for someone to visit you to complete the 'support worker information' section.
