Changes to Westbourne Park Road
Following a consultation in autumn 2010, we are no longer providing a day care centre for people with physical disabilities at 42 Westbourne Park Road.
If you have high support needs
You will have already had an assessment with a care manager from the council.
If you are eligible, you will be given a weekly sum of money or personal budget to buy your care.
An organisation called Hammersmith and Fulham Action on Disability will contact you to help you develop a support plan to decide how to spend your personal budget. Your support plan should be in place by early September.
If you are not eligible for social care
The centre staff will help to identify your options after the centre closes.

Personal budgets
Personal budgets are a new way of paying for your care. A personal budget is money that is paid either into your bank account or used to pay for services on your behalf by the council or another advocate.

Five steps to a personal budget:
1. You will have an assessment with your care manager to find out if you qualify to receive a personal budget and how much you need to pay for your care
2. If you are eligible, we will help you to do a self-assessment to find out the type of care you want and need
3. This will be put into a support plan which sets out how you will use your personal budget
4. You will be given a personal budget to pay for the care highlighted in your support plan. You should keep this in a separate bank account.
5. You will be given a directory of services to help you choose your care
