Carers Assessment
If you provide a regular and substantial amount of care for someone aged 18 or over, you can ask for a carer's assessment.
A carer's assessment means looking at your situation to see if you are entitled to any services that could make caring easier for you.
The assessment can be carried out at your home or at the home of the person you are caring for. The assessment is about you, and the person you care for does not need to be present.
You can ask a friend or relative to be with you during the assessment, if you want to.
If there is more than one carer providing regular care in your household, you are both entitled to an assessment.
Joint Assessment
We can carry out an assessment of your needs as a carer at the same time as an assessment of the person you are caring for, as both of you may benefit from support services.
Individual Assessment
If you would prefer to have an individual assessment which focuses purely on your needs as a carer without an assessment of the person you care for, this is also possible.
Requesting an assessment
Simply call us on 020 7641 1175 (Mon-Fri 9am-5pm) for an initial discussion about your caring role and what your needs may be. Following that, we will arrange an assessment if appropriate. This will usually take place at home.
Preparing for the assessment
To prepare for an assessment, you can read and fill out the following self assessment form.
The assessment is an opportunity for you to tell the social worker what impact caring has on you. So it may be a good idea to make a list, or keep a diary, of everything you do to look after the person you care for.
