Apply for a copy of a certificate

You can apply for a copy of a certificate:

• In person at the Westminster register office.
  
  
• By post (download and print page one of the application form - you need Adobe Reader to access the form). You will need to enclose a cheque or postal order made payable to 'Superintendent Registrar' and a stamped addressed envelope (or add £1 on to the fee for postage and packaging). 

If you are applying from overseas, please call us on 020 7641 1162 during our opening hours so we can take payment over the phone by credit/debit card.
