We will do all we can to support you to live as independently as possible in your own home. However, if your care needs increase to the extent that you require 24 hour care in a residential or nursing home, we will discuss this with you and reassess your needs.
With your permission, we will usually consult with others about this before any decisions are made - for example your family, carer, your GP and any other people who have been involved in your care at home.
Decisions regarding a move to a long-term care home will be made with you. Where practical, we will always take your wishes into account about where you would like to be, and we will try to assist with finding a suitable home for you.
If you move to a residential or nursing home you will be given a Care Plan which explains how the staff in the home will help you. Your care manager will keep in contact with you to see that you are settled in.
If you go into residential or nursing care funded by Social Services we will visit you to review your care needs. We aim to do this at least annually
Yes, you will be assessed to pay for your accommodation. We may agree to fund all or part of your placement after your savings, income and possibly property value have been taken into account. Any charges will be explained to you and the amount you will have to pay towards the cost of your care will be based on national guidelines (the Charges for Residential Accommodation Guide Regulations – known as CRAG).
